Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under Section 2799B-6 of the Public Health Service Act, health care providers are required to inform patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services in writing prior to receiving services upon request.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.  You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.For questions or more information about your right to a Good Faith Estimate,call 1-800-985-3059 visit www.cms.gov/nosurprises

 

Cancellation & No-Show Policy

Each patient is required to maintain an active and valid credit card on file to bill automatically for any payments due at the time of service. Failure pay fees without an agree upon payment arrangement will result in cancellation of appointment and other services.

Each appointment time is reserved specifically for you. Therefore,it is necessary to charge a late cancellationfee of $80 for appointments cancelled with less than 24 hours in advance. No show fee is $80.Please be aware that your insurance will not pay for a missed appointment or late cancel fee.
